| Additional Description |
The State Innovation Model (SIM) Program Management Office (PMO) is seeking practice transformation services to support the implementation of our Advanced Medical Home Pilot initiative. Our proposed pilot must test transformation methods aimed at improving practice and patient experience, allowing for flexibility in the application of these methods so that participating practices can help us to identify the optimal approach, as well as those methods or features that may not work or may have a negative impact on patient and practice experience.
The PMO seeks to implement this contract on or before January 1, 2015. This is a competitive procurement. The PMO has identified a number of individuals who will participate in the RFP development or selection process. The anticipated maximum award is $650,000. |
