| Additional Description |
The Children’s Trust Fund (CTF), a Division of the State of Connecticut Department of Social Services (the Department) is seeking submissions from individuals qualified to provide consulting services to promote child development surveillance and the Help Me Grow program. Consulting services will be provided on a full-time basis (40 hours per week) to pediatric and family medical practices throughout Connecticut. The goal of this project is to increase the use of the Ages and Stages Questionnaires (ASQ) and Parent Evaluation of Development Status (PEDS) within medical practices so that more children at risk of developmental problems will be identified early and referred for services. Only individuals that meet or exceed the minimum qualifications set forth in the RFQ are eligible to submit responses. Private provider organizations (defined as nonstate entities that are either nonprofit or proprietary corporations or partnerships), Connecticut State agencies, and municipalities are ineligible to participate in this procurement. |
