The State Innovation Model (SIM) Program Management Office (PMO) announces the release of a Request for Applications (RFA) for the Advanced Medical Home (AMH) Vanguard Program. Eligible applicants are Advanced Networks (ANs) with primary care practices who are not currently recognized under an existing national medical home standard. The AMH Vanguard Program will provide transformation support aimed at improving practice and patient experience. Selected applicants will receive the services of Qualidigm and Planetree to achieve the National Committee on Quality Assurance (NCQA) Person Centered Medical Home (PCMH) Recognition and Planetree’s Patient-Centered Bronze Recognition. Practices will receive practice transformation support for up to 15 months, as well as discounted 2014 NCQA PCMH application fees. NCQA PCMH recognition will prepare practices to participate in new payment and care delivery reforms, including the Medicaid Quality Improvement and Shared Savings Program (MQISSP), the RFP for which is expected to be released by the Department of Social Services (DSS) in June of 2016. DSS will only attribute Medicaid beneficiaries that are receiving services from Advanced Network practices that have NCQA PCMH Recognition (Level 2 or 3) by the program start date of January 1, 2017.
The PMO seeks to implement the AMH program in two cohorts of up to 25 practices each, the first beginning in March and the second in April. Applications for the first cohort are due by March 14, 2016 at 3:00 p.m and for the second cohort by April 11, 2016. We encourage submitting an application as soon as possible. The Request for Applications is available in electronic format on the PMO website at: www.healthreform.ct.gov. The PMO may close the application process early if it reaches its recruitment target.
