| Additional Description |
DOH is seeking proposals from eligible 501(c)(3) nonprofit organizations and municipalities to provide at least one program for people who are homeless or at risk of homelessness along the Connecticut continuum of services. . This continuum includes coordinated access services, shelter diversion services, shelter operations, rapid re-housing services, new permanent supportive housing services or youth transitional living programs. Services will be provided within a specified Coordinated Access Network (CAN). |
