| Additional Description |
The State Innovation Model (SIM) Program Management Office (PMO) is seeking consultation services to support the implementation of the Connecticut Healthcare Innovation Plan. The goal of the Innovation Plan is to implement a whole-person-centered health care system that improves health and eliminates health inequities; ensures superior access, quality, and care experience; and improves affordability by reducing healthcare costs. The PMO requests proposals from qualified applicants to provide consultation services. The PMO seeks a team with expertise in change leadership and management and specific responsibilities that include but are not limited to (a) meeting facilitation, (b) materials preparation and communications to support the active engagement of providers, consumers and other stakeholders, and (d) program organization and management. The PMO intends to implement this consultation contract by September 2014 for a contract period ending June 30, 2015, with the option to extend. |
